James Leo “Zack” Zack, 63, of Middlebury, passed away Tuesday, March 22, 2016 at IU Health Goshen Hospital.

He was born March 14, 1953 in Ponca City, Oklahoma to Chester and Lucille (Kerns) Zack.

On June 10, 2000 he married Karen Sue (Snook) Duncan in Elkhart.

She survives along with two children, Phoenix Zack and Jamie Duncan; 12 grandchildren; and a great-grandchild.

He was preceded in death by his parents and two sisters.

Zack enjoyed being around his grandchildren.

Friends may call Saturday, March 26, 2016 from 3 until the 5 p.m. funeral service at Yoder-Culp Funeral Home.

Cremation will follow the service.
